Massachusetts Medication Pass Demonstration

the following is information I want you to be aware of regarding the medication administration component of your skills test remember you have ten minutes to give 1 countable medication to one individual you will not be administering a liquid medication then that is a tablet it's countable in blister packaging and may or may not have a C stamped on the package please be aware the medication in the blister pack may have been popped out of sequence by previous candidates if so you will see a sticker on the package calling your attention to this this means you must count each tablet separately and cannot rely on the number blisters this is not an attempt to trick you the tester will read a set of instructions the instructions include being asked if you want to be informed when five minutes remain or if you want to complete the process uninterrupted after those instructions are read is when you're 10 minutes begin at the end of the instructions the tester will ask you if you have any questions as once the test begins their run allowed to answer questions at this point you may ask for a small piece of paper to write notes on the paper will be collected at the end of the test the test who will assume you've washed your hands before and after the process if you forget to mention that you've done so you do not fail verbalise when comparing the five riots during the three cross-checks remember document in both the medication book and the count book if you forget to sign the bottom of the med sheet you do not fail however if you initial the med sheet before administering the medication you will the remainder of this presentation is a demonstration of the medication administration process welcome to your medication administration test please follow along with me while I read the instructions I will be observing and recording what I see you demonstrate D&S diversified text technology staff in Findlay Ohio will officially score your test I do not decide whether you have passed or failed this is the medication administration portion of the skills test you are being asked to administer a medication to an individual according to the scenario which I will read to you listen carefully you will also be given a copy of the scenario for reference the medication you are administering is a comfortable substance following the instructions on a scenario you like to administer the proper medication to the individual and document the administration using the appropriate medication sheet and control substance calm sheet while you're administering the medication in order for me to know if you have completed each of the three checks for the five rights you are expected to clearly demonstrate to me that you are following the chest if it is not clear to me you will not be credited with completing the checks you will have ten minutes to complete this task would you like to know when you have five minutes remaining No here is your medication book in the new meditation book each individual has a designated area in that area you will find the healthcare provider order the medication administration sheets and the medication information sheets this is the medication box here you will find the medications for each individual each individual has their own draw here we have the supplies that you need to give your medication and this is the control comfortable subjects book that you will use today is September 30 2012 please administer by leaves a p.m. medication on September 30 2012 document the administration on a medication administration sheet and complete the medication comp record it would be helpful if you verbalize each of the five rights as you demonstrate the three checks you need to tell me when you are finished I cannot answer any questions once we begin the test do you have any questions at this time no I don't okay you may begin when you're ready okay okay I'm going to start my med pass I have all of my supplies I've washed my hands and I'm going to unlock my med cabinet according to my scenario I'm going to give viola his medications on September 3rd at 8 p.m. in order to figure out which medication I'm going to give I need to open my med book I locate viley and I can see that on September 3rd at 8 p.m. viley is going to receive klonopin so now I locate by Lee's Med box and I locate the correct medication I have the Quan a pen now I'm going to locate the medication page in the controlled substance book I do this by first turning to the index I can see that vial ease klonopin is on page 2 so I have the correct page open and I'm ready to start Chuck number 1 check number 1 is between my health care provider order and my pharmacy label I have vilely klonopin one milligram by mouth two times daily viley klonopin 0.5 milligrams take two tablets by mouth two times a day I've completed check one I'm now going to start check two between my pharmacy label and my med finally klonopin 0.5 milligrams take two tablets by mouth 2 times a day my med sheet viley klonopin 0.5 milligrams is my strength my dose is 1 milligram the amount is 2 tablets by mouth twice a day I'm going to give the 8 p.m. dose on September 3rd I've completed check number 2 which means I can now prepare my medication into the cup and make sure I have the two tabs I'm now going to start check number 3 by lee klonopin 0.5 milligrams take two tablets by mouth two times a day my med sheet by Lee klonopin 0.5 milligrams my dose is one milligram the amount is two tablets by mouth twice a day I've completed three checks I can now administer the medication can you go vitally here's some water for you and make sure I observed for a swallow and then I'm going to do my look again I can now document the medication administration I do this by putting my initials in the corresponding date and time box and then I sign the bottom for the signature list I now come to the count book its September 3rd of yr or 12 the time is 8 p.m. the route was by mouth I had 24 on hand I administered 2 tabs I write out the word 2 and my amount left is 22 I sign my full name and I'm done with the documenting I now need to secure my medication and I make sure that I lock the cabinet I am going to wash my hands and I always make sure I continue to observe for the effects of a medication and I've completed my Methos you have     Verbalized when you're doing the checks "I am now going to start check 2" etc

    The first time you do check 1 you need to say all the 5 rights and when you do the frequency immediately after you say … AND ACCORDING TO THIS SCENARIO I WILL ADMINISTER THIS MED AT 7/YR at 9 pm and point to the box you will do the check ".

    Also verbalize when you are doing the silent look again. And pretend you're looking and point while ur saying it in ur head.

  5. Haha, one lousy pill. Try medicating 27 patients during a survey, patients who average 15-20 meds each including eye drops, nebs, and inhalers.

  6. Actually it was the computer test I didn't pass . Lots of trick questions and "the best answer" questions which I hate as two answers are relatively same and it's a 50/50 shit if you pick one over other. Is there anything online for actual answers to Massachusetts MAP test ? I know the EMS state test was all on line made into cue cards. Boy did that help

  7. this site has improved but didn't see her check the med in the blister pack to verify the number on the sheet and in the pack.

  8. Does anyone knows if it would be an automatic fail if I forgot to say "looking for side effects at the end"?

As someone who failed this test several times I can tell you without a doubt that this person would fail under many D&S testers.  Not once did she display the 5 rights, right person right med, right dose, right route, and right TIME.  Time is a specific time of the day, 8PM, 7AM, 4PM.  Frequency and time are not the same thing, saying that they take a med 2 times a day without mentioning what times those are and that one of those times is right now will fail the test. 

    It's their easy out to make more money through failing participants.  If you want to pass this test without question you should say something along the lines of, "two times a day and 8PM is one of those times, and it's 8PM right now".  Anything less than that does not "prove" you verified the 5 rights before administering the med.

    Remember that frequency (two times a day, 3 times a day, once daily) does not verify that you are giving it at the right time.  Frequency is not one of the 5 rights, time is, don't let them waste your time with a semantic issue, clearly tell them that it is X O'clock and X O'clock is when the individual should take the med.

  10. I am taking the test today and was reviewing demo. after one time it stopped working and goes right to a fuzzy blank screen!!!!!!! yikes!!!!

  11. Shouldn't she have done a count prior to the med pass to make sure that the count was right? I mean if you go and give a med then find out the count is wrong aren't you causing issues for your program?

  12. at least fix the black  screen( AN error occurred, please try again later. learn more) and i am taking a test in the morning i comment my results if i passed. really i think its money making. and this goes for the staff that are on the phone with me that is taking a test same time. i comment this to people when waiting to take the test.

  13. I am sick of this site. This goes for everybody taking the test. This site always have an error occurred I tried my own computer. family members computer. going to a library near by in one day and the next. how do expect people to study through this site. its all money making. this the worst site. needs to be fixed. ALL money making i agree.

    D&S flunks about 50% of the med admin oral exam takers. The exam instructors are speficialyl told that this is about how many people should pass/fail within a typical random sample.

    The reason for this is that D&S makes $150 for every Fail they pass out. Specifically they profit the $150 off of the re-test. Thus the instructors are under lots of corporate pressure to pass out flunks, so most of them do. They get away with it because the state guarantees their business, so they have a de facto monopoly over this narrow mini-market. If you failed and you know you did everything right, don't take it personally. You probably did pass but the $150 is worth more to them than your time or job security.

  17. I have been a MAP trainer since 1998. When the curriculum changed, the FIRST check became the HCP order TO the pharmacy label AND the pharmacy label to the med sheet. So, she's doing check one wrong. Also, she never identified the individual. And, she never "double locked" her countable substance at the end of the test when she put her meds away.
